Team captains should submit this form on behalf of their team. Tournament details will be emailed out the night before it begins.

Please note that every member of your team must meet the eligibility requirements of the specified division. (you can't add a high school player to a middle school team, a male to a female team, or a middle schooler to a rec team.)
We will use this to help balance pool play, if necessary.

Waiver

Ultimate frisbee is a physically demanding sport which includes running, jumping, and changing directions. There is a risk of contact with other players and injuries could include sprains, bruises, contusions, concussions, broken bones, or even death.

By registering you accept the risks of the sport and assume liability for your injuries and to hold Cache Valley Ultimate and it’s volunteers free of liability.
